How to apply for CCAP in Chicago
In Cook County, CCAP applications are handled by Illinois Action for Children. This site is not affiliated with them — these are just the steps, in plain English.
- Check if you may qualify. CCAP is for families who are working or in school/training and meet income limits. Read the overview at Get Help with CCAP.
- Pick a provider that accepts CCAP (use our directory, then call to confirm).
- Get the forms from the CCAP forms page. Your provider fills out their part.
- Gather proof: ID, pay stubs or school schedule, and your children's information.
- Submit online, by mail, or at a drop box. Questions? Call 312-823-1100.
